Why do my feet ache so badly when I stand up and walk, the outer foot, the heel and the toes? The pain has been very bad for about 4 months now.

- I have been to three podiatrists, have a customized insole, have tried met pads, and nothing seems to help. Problem is in the forefoot and is apparently caused by capsulitis. Forefeet and toes feel swollen during the night, and it is extremely uncomfortable to walk almost all the time, except when I use some cheap sandals that have adapted to the shape of my feet. I would pay some serious money to a shoe advisor who could direct me to shoes that might be useable. I can barely walk after being in even expensive shoes for two hours. Is there such a person who does one-on-one consultations?? Any light you can shed would be helpful.
- I've had 3 separate occurrences of spontaneous bruising of my right foot. It starts with a feeling of standing on a large rock, with hard swelling on the bottom of my foot, and turns into a large area of bruising, encompassing the side and top of the foot. I have flat feet and have always had flat feet. I have always had some foot pain but not anything that slowed me up. I do know as a kid ice skates would hurt like hell after a fairly short time but I attributed that to the arch in the skate forcing my flat foot into an unnatural position. The wear pattern on my shoes is classic pronation wear. I am a runner and in the past have done a fair amount of races including a marathon in 1988. The last few years my feet started hurting after running fairly short distances (3-4 miles or long walks probably 10-15 miles). These are normally when we are traveling and wandering around a new city. The pain was/is bad enough to have me hobbling about the next day and to inhibit me from running. I had some relief from using motion control shoes. A year ago I bought some Birkenstock fitted 3/4 arch supports, which seemed to help more than the motion control shoes. But I still have foot pain the night and the next day if I am on my feet all day or run just a few miles. The pain is focused on the inside center of both my feet at the top of the arch and up and inch or two. Any ideas as to what I could do to mitigate the pain? I am 55 years old, 6 ft 2 in 180 pounds. I always ran about 165 lbs until 7-8 years ago when I added weight lifting to my routine and I put on 15 pounds of upper body muscle. I am in excellent general health and not on any medications. I do take some nutritional supplements and have for over 30 year: presently C, E, a multi-vitamin, low dosage aspirin, fish oil, and melatonin.
